Obtaining the Form and Reviewing the Instructions
a. Go to Sunbiz and navigate to the “Forms & Fees” tab.

c. Beneath “Florida LLC Forms,” click on “Amendment (PDF) to access a form called the Articles of Amendment to the Articles of Organization.
Filing the Articles of Amendment
a. Fill out the Cover Letter with your company’s details.
b. On the first page of the application, in Section A, you’ll see a line stating, “If amending name, enter the new name of the limited liability company here.” Here, you’ll enter the new name of your LLC and it must include the words LLC or Limited Liability Company at the end of your name. c. If there are no other changes, leave the rest of the filing blank.
d. Have a Member or Authorized Representative of the LLC sign and date the last page.
e. Print the signed and dated document and mail it along with a $25.00 check made payable to the Florida Department of State to:
